<br />For Botswana citizens, enter 001 in the shaded area under column A6. For citizens of other countries, write the name of the country in the unshaded area and the code for that country in the shaded area beneath. The codes for most Southern African countries are shown on the questionnaire. If a person is a citizen of a country not listed, look up the code in the Locality Coding/Country List.</p>
