This is the first Household Health Coverage Survey conducted for the Five-Year Global Fund Evaluation, as part of the District Comprihensive Assessment activity.
This survey was conducted in the scope of the Global Fund Five-Year Evaluation.
Purpose
(1) To provide population-based data on knowledge of TB and HIV/AIDS; sexual behavior; utilization of services related to HIV testing and maternal and child health; and coverage of IRS and bednets
(2) To provide basic information on health expenditure

National - at district level
The survey covered all de jure household members (usual residents) in the selected households of the selected enumeration areas in the eight sampled districts , all women aged 15-49 years resident in the household, and all children aged 0-59 months (under age 5) resident in the selected household.

6625 households were selected from 8 districts and 30 clusters, 25 households per district.
Selection process: 8 districts were purposefully selected based on geographic criteria (one per region), and urban/rural to reflect national urban/rural mix.
Stratification: stratified by district
Stages of sample selection: purposeful selection of 8 districts, clusters selected PPS (probability proportional to size), households in clusters were sampled systematically after household listing was updated.
Design omissions: In 'City district of Lilongwe', the city and rural were separated as two districts. In the city 25 clusters were sampled with 25 households selected in each cluster. In Lilongwe rural 30 clusters were sampled with 25 households in each cluster. That is essentially we had 9 districts.
Level of representation: district level
Strategy for absent respondents: no replacement
Sample frame: Original sample frame of clusters is based on the 1998 Malawi Population and Housing Census; household listing in selected clusters prior to systematically selecting the households.
Household response rate 94% and woman response rate 94%.
The household weight is the inverse of the household selection probability multiplied by the inverse of the household response rate of its household response rate group. The woman's weight is the household weight multiplied by the inverse of the individual response rate of her individual response rate group.
Note the HH weight variable is in the QHSECOVER record, and the woman weight variable is in the QSECOVER. These weights are not merged with respective household and woman records.
Questionnaire modules were based on the standard DHS, including a HH and a woman questionnaire.

Interviewing was conducted by teams of interviewers. Each interviewing team comprised of 4 female, 2 male interviewers and a team leader, and a driver. Each team used a 4 wheel drive vehicle to travel from cluster to cluster (and where necessary within cluster).
The role of the team leader was to was to coordinator field data collection activities, including management of the field teams, supplies and equipment, maps and listings, coordinate with local authorities concerning the survey plan and make arrangements for accommodation and travel. Additionally, the team leader assigned the work to the interviewers, spot checked work, maintained field control documents, and sent completed questionnaires and progress reports to the central office.
The team leader was also responsible for reviewing each questionnaire at the end of the day, checking for missed questions, skip errors, fields incorrectly completed, and checking for inconsistencies in the data. The team leader also observed interviews and conducted review sessions with interviewers.
Responsibilities of the team leader are described in the Instructions for Supervisors, together with the different field controls that were in place to control the quality of the fieldwork.
Field visits were also made by a team of Senior supervisors on a periodic basis during fieldwork. The Senior supervisors of National Statistical Office also made several visits to field teams to provide support and to review progress.
The trainning of enumerators and team leaders together with data processors was coducted for ten days in mid december 2007. The interviews were conducted in local language. The enumerators were selected from a pool of experianced enumerators who had just finished conducting the Agriculture Census to ensure good quality of the data collected.
Data editing took place at a number of stages throughout the processing, including:
a) Office editing and coding, verification of completed questionnaires
b) During data entry (scanned questionnaires also pass through verification process)
c) Structure checking and completeness out files in SPSS format

The Global Fund 5-year impact evaluation data is available to outside researchers as licensed data files. These files can be accessed though the public web site and require user registration. There is a two step procedure required to access the files:

The names of the facilities have been suppressed. However, some of the files contain geographic references that could be used to re-identify the facility. For any information required to validate the name of the facility, please contact the appropriate country authority.
Household:
Household data is recoded and no direct identifiers are available. No special conditions apply for access to the recoded data.
