Definition
To fill the time of visit interview information, please fill out the section to each visit. For each visit given the opportunity as much as three visits. For each visit write down the date, the clock starts and ends at a visit. Do not forget, the write clock starts and ends at the interview shall distinguish between 08.00 am and at 08.00 the night (to be written at 20.00). If the interview was conducted in the afternoon at 12.00 write a complete, eg 14:15 hours.
