Interviewer instructions
The actual date of birth should be entered in the boxes provided, of which there are two for the day, two for the month and four for the year. If the day is less than 10 enter a zero in the first box. The months are numbered 01 for January, 02 for February, ¼09 for September, 10 for October, ¼... and 12 for December.
