<div class="i1">(i) Give the usual residence of [the child's] mother at time of birth, not the location of the hospital or the nursing home where [the child was] born.<br />(ii) If [the child was] born in any part of Ireland (including Northern Ireland), give the name of the county of birth.<br />(iii) If born outside Ireland, whether now resident in or visiting Ireland, state the country of birth.</div>
