<p>Fill in the oval in option 2, "no", if the respondent lives permanently in a district other than the one in which you are registering. Proceed to question 5A. In which district and province does he/she live permanently? and write down the name of the district and province in the corresponding boxes.</p>

<p>Fill in the oval of option 2, "no", if the respondent lives permanently in another country. Then, in question 5A, write down the name of the country in the box corresponding to Country.</p>
