Interviewer instructions
Using Appendix 1 of the manual, write down the district of birth for each member of the household who were born in Kenya, or country of birth for each member of the household born outside Kenya. You should understand that the first two digits in the district codes represent the province and district within that province, respectively. The last two codes indicate whether one was born in urban or rural areas of Kenya. Therefore each place of birth within Kenya has to be represented by a combination of 4 codes. For example, write the district "Kisumu" in column A11, and put code "6201" in cell 26 through 29 to show that a member of the household was born in Kisumu district within Kisumu municipality.
If on the other hand the respondent was born in a rural area within Kisumu district, write the district "Kisumu" in column A11, and put code "6200" in cell 26 through 29. The last two zeros indicate rural areas, i.e. not born in any of the towns listed under a district. If born outside Kenya write down the name of the country in column A11, and put the country's code in cells 27 through 30 by use of Appendix 1 of the manual. It should be noted that the listed districts and towns are those covered by NASSEP III sampling frame; i.e. all districts in existence in 1992; and all towns with a population of 10,000 or more persons as given by the 1989 Population Census, plus any district headquarters with less than 10,000 persons.
