Literal question
How many times did [NAME] fall sick during the last 30 days?
Interviewer instructions
This question seeks to find out whether the respondent was ill or injured during the last 30 days before the date of the interview. For respondents who have not been ill or injured during the last 30 days, you should probe to make sure that the respondent has not forgotten any recent illness or injury whether treatment was sought or not. In case one fell sick during the last 30 days assign code 1 for "yes" else record 2 and skip to the next person.
