<div class="i1">(i) These questions should be answered for all persons who are now usually resident in Ireland (Republic) and who previously lived elsewhere for a continuous period of 12 months or more; or who were brought to live here before their first birthday.<br />(ii) On Question 12, please distinguish Northern Ireland separately.</div>
