A mark that corresponds to the one of the mentioned variant of response is written down.

Yes is marked to those who have always lived in the settlement since birth. This does not depend on residence permit and its type (permanent or temporary) and is not broken by address changing within the one settlement or rural area of the one administrative region. It is also not broken by departure from the settlement to go military service and abroad on a business trip; all departures from the settlement for the period not more than 12 months.

No is marked to persons who live in the urban settlement not since birth including those who was born here but left for permanent residence (for the period 12 months and more) to another urban settlement or rural area.

No is marked to persons who live in the rural area of the administrative region not since birth including those who was born here but left for permanent residence (for the period 12 months and more) to another urban settlement or rural area of another administrative region.

For persons who answered no you should fill in the following questions:

a. if no, please indicate the year from which you continuously live.

You should indicate a year from which a respondent continuously resides in this city or rural settlement of the administrative area. To persons (both born in this settlement and those who live in it since birth) who went from this settlement to another place of residence for a period of 12 months or more you should indicate the year when they returned to this settlement for the last time.

Also you should make a mark if the time of moving is within the period from the 5th of December 2000 to the 5 of December 2001.

To those whose place of previous residence was in Ukraine, one of the names is indicated: the Autonomous Republic of Crimea, Vinnytska, Volynska, Dnipropetrovska, Donetska, Zhytomyrska, Zakarpatska, Zaporizka, Ivano-Frankivska, Kyivska, Kirovohradska, Luhanska, Lvivska, Mykolaivska, Odeska, Poltavska, Rivnenska, Sumska, Ternopilska, Kharkivska, Khersonska, Cherkaska, Chernivetska oblasts, Kyiv and Sevastopol cities. For those who came from another country you should write down its name.

c. type of settlement a person arrived from.

A mark that corresponds to the one of the mentioned variant of response is written down.
If a respondent came from a city or urban-type locality in this settlement, then you should mark urban; if from a rural settlement - rural. You should not take into consideration administrative and territorial changes that took place after responder's departure from his/her previous place of residence. For example, if the rural settlement, where a respondent lived before moving in a city, was converted to urban settlement or included into the territory of a city, you should mark rural in point b.

You should make a mark for people who are refugees or deportees.
Refugees are foreigners (or stateless persons) arrived in Ukraine who had to leave the territory of the country of their citizenship (or territory of the country of their permanent residence) due to well-founded fear of being persecuted for reasons of race, nationality, religion, membership of a particular social group or political opinion and cannot or do not wish to be under protection of that country because of mentioned concerns.

Furthermore, a decision on provision of refugee status to those people was made according to the procedure and under the conditions stipulated by the legislation of Ukraine.

Deportees are people arrived in Ukraine (and their family members - wife / husband / children, grandchildren) who were forcibly evicted beyond the current territory of Ukraine from their homes and in respect of them the repressive policy with confiscation of property, setting regime of special settlements, restriction of political, civil, social, economic and cultural rights was carried out and who are returned to their homeland.
