HCMAF4 is combined with HCMAF1 (public comprehensive health center/polyclinic), HCMAF2 (public basic health center), and HCMAF3 (public health sub-center) in HCMPUBHC (all public health centers and health posts) for the 2015 Afghanistan sample.
