* Caroline van den Berg: even though WHO published data as of 2000, the 2002 data must be considered an update of the 2000, as opposed to an additional year.
* The reason is every data point is estimated through a regression with the data available from different hh surveys. If a new hh survey becomes available, new estimates are calculated using all data available per country.
