There are 18 sections in the questionnaire used in the census of manufacturing establishment 2001-2002. At the right down corner of the cover page, a four-digit unique identification number was designed by combining district code with serial number of enumerated establishments of corresponding district. Hence constructed unique identification number is defined as a variable "form no" which stands for "Form ID".
Four considerations to be given during interview are kept just after the cover page.
The first four sections of the questionnaire are introductory sections. The first section is designed to record the introductory
information of the establishment including name, spatial address, registration number, phone number, post box number, fax number, date of operation and contact address. The second question is related to name of which enterprise belonging to Establishment.
The third and fourth sections have one-one closed question having 5-6 options. They will give the legal status and ownership status of the establishment respectively.
Major industrial activity is kept in fifth section with NSIC code at four digit level.
The sixth section deals with employment and emolument. Fuel and industrial services are in section seven and eight respectively. At the section 9 and 10 are recorded the cost of raw materials purchased and value of shipments by country and CPC. Sections 11, 12, 13, 14 and 15 possess industrial receipts, stocks, non-industrial cost, indirect taxes and non- industrial receipts respectively having 4, 7, 14, 5 and 7 sub-sections. The section 16 records fixed assets of establishments at the beginning of the year, addition of fixed assets during the year and sale, loss and depreciation of fixed assets during the year. Sections 17 and 18 have questions for utilization of capacity and problems faced by the establishments. |
