Description CPH Form 1 - Listing Booklet
This form is a booklet used to list the buildings, housing units, households, and the Institutional Living Quarters within an EA. This form also records other important information such as the name of household heads and name and type of institutions and their addresses, population totals, and counts of males and females.

CPH Form 1 is a four-page listing booklet. The first page of this form contains four sections, namely:

Title Panel: Contains the form type, authority and confidentiality clauses, office name and the title of the census and CPH form, serial number, page number, NSCB approval number, and expiration date.

Identification Panel: Consists of certification portion and geographic identification.

Listing Record: Contains 11 columns which consists of the line number, day of visit and callback indicator, BSN, HUSN, HSN, institutional serial number (ISN), name of household head or name/type of institution and address, total population count, population count of male and female and the remark/s portion.

Page Totals: This portion is found at the bottom of the form. It is used to record the total number of buildings, housing units, households and institutions, and the total male and female population in the households and institutions listed on the page.

The second, third, and fourth pages of this form also consist of Listing Record and Page Totals.

Description CPH Form 2 - Common Household Questionnaire
This form is used to record information about the common or nonsample households. This questionnaire gathered information on the demographic and socio-economic characteristics of the population. It also contains questions on the housing characteristics.

CPH Form 2 is the form used to enumerate all non-sample households. It is a four-page questionnaire which served as the record of the basic characteristics of the household population and the housing units they occupy.

The first page (page 2A) of this form contains the following:

1. Title Panel
2. Identification Panel
3. Interview Record
4. Household Definition and Membership

The second and third pages (pages 2B and 2C) of CPH Form 2 contain the Population Census Questions. These pages consist of 14 columns containing the questions on demographic, social, and economic characteristics of the household.

The following are the Population census questions (P1 to P8, P11 to P13, and P17 to P19) which gathered data on the demographic and socio-economic characteristics of the population.

P1 - Name of Household Members
P2 - Relationship to Head
P3 - Sex
P4 - Date of Birth
P5 - Age
P6 - Birth Registration
P7 - Marital Status
P8 - Religious Affiliation
P9 & P10 - Citizenship
P11 - Ethnicity
P12 - Disability
P13 - Functional Difficulty
P14 - Residence 5 Years Ago
P16 - Highest Grade/Year Completed
P19 - Overseas Worker

The bottom of the page 2B contains a question on whether there are household members that are not listed and an instruction on what to do if there are more than eight members in the household. It also contains the codes for relationship to household head. The codes for highest grade/year completed, meanwhile, are found at the bottom of the third page (2C).

The fourth page (page 2D) contains the seven items on housing characteristics and a remarks portion where you can write any necessary information that can be of help to you and your supervisor when scrutinizing the questionnaire.

The following are the Housing census questions (B1 to B5, D1 and H8) - which gathered data on the characteristics of the buildings and/or housing units.

Description CPH Form 3 - Sample Household Questionnaire
This form is used to record information about the sample households. This questionnaire contains ALL questions asked in CPH Form 2 PLUS additional population questions. Moreover, there are additional questions on household characteristics.

CPH Form 3 is the form that used to enumerate all sample households. It is a four-page questionnaire which served as the record of the characteristics of the household population as well as their housing unit & household characteristics.

Like CPH Form 2, CPH Form 3 contains questions on population and housing. However, CPH Form 3 contains additional population questions (P15, P17 toP18, and P20 to P27), housing questions (H1 to H7 and H9 to H11), and household questions (H12 to H15).

The first page (3A) of this form contains the following:

2. Identification Panel
3. Interview Record
4. Household Definition and Membership

The second page (3B) of the questionnaire contains the Population Census Questions (P1 to P13). Questions on household size, specifically on whether there are household members not yet listed and whether there are more than 8 household members and codes for relationship to household head are found at the bottom of this page.

The third page (3C) contains other population census questions (P14 to P27) including the folowing sample population items:

P17 - School Attendance
P18 - Place of School
P20 - Usual Occupation
P21 - Kind of Business or Industry

This form is used to record the characteristics of population residing in Institutional Living Quarters such as hotels, hospitals, welfare instiitutions, corrective and penal institutions, convents and seminaries, military camps, logging and mining camps, oceangoing vessels, and refugee camps.

Description CPH Form 5 - Barangay Schedule
This form is used to record the characteristics of each barangay, kinds of establishments and service facilities in the area or within some specified distance from the area, and travel information from town/city proper to the barangay.

CPH Form 5 is a four-page questionnaire used to record the barangay characteristics and facilities.

The first page of the questionnaire contains the following:

Title Panel

· NSCB approval number and the expiration date;
· title of the questionnaire;
· serial number; and
· authority and confidentiality clauses.


Geographic Identification Panel

· geographic location of the barangay to be visited;
· interview record; and
· certification by the TS and his/her supervisors regarding the veracity and accuracy of the data collected.
