    The WHO STEPwise approach to surveillance (STEPS) is a simple, standardized method for collecting, analysing and disseminating data on noncommunicable diseases (NCDs) and risk factors. Data are collected on the established risk factors and NCD conditions that determine the major NCD burden, including tobacco use, harmful use of alcohol, unhealthy diet, insufficient physical activity, overweight and obesity, raised blood pressure, raised blood glucose, and abnormal blood lipids. Data from STEPS surveys can be used by countries to help monitor progress in meeting the global voluntary targets related to specific risk factors such as tobacco, alcohol, diet and physical inactivity. The tobacco indicators from STEPS can be used to evaluate and monitor existing tobacco-control policies and programs.

    This is the second STEPS conducted by Sudan.

    Abstract

    The STEPS survey of noncommunicable disease (NCD) risk factors in Sudan was carried out from February to December 2016. Sudan carried out Step 1, Step 2 and Step 3. Socio demographic and behavioural information was collected in Step 1. Physical measurements such as height, weight and blood pressure were collected in Step 2. Biochemical measurements were collected to assess blood glucose and cholesterol levels in Step 3. Furthermore, urine was collected in one region in Khartoum state only with a sample representative to this region. The survey was a population-based survey of adults aged 18-69. A four stage stratified cluster sampling was used to produce representative data for that age range at country and regional level. The total sample size was 8145 participants; the response rate was 95% in step 1 & 2 and 88% in step 3. All results were weighted by gender and age. A repeat survey is planned after implementation of updated mutli-sectoral action plan 2019-2023 if funds permit.

    The following topics were included in the survey: tobacco use, alcohol consumption, diet, physical activity, history of raised blood glucose, history of raised blood pressure, history of raised total cholesterol, history of cardiovascular diseases, lifestyle advice, and cervical cancer screening. Additionally, the following measures were taken: blood pressure, height, weight, waist circumference, heart rate, fasting blood glucose, total cholesterol, HDL cholesterol, 24hr and spot urine. Finally, the following optional modules were included: tobacco policy and oral health.

    Sampling Procedure

    A multi-stage cluster sample of households. One individual within the age range of the survey was selected per household.

    Response Rate

    95% in step 1 & 2 and 88% in step 3

    Weighting

    Analysis weights were calculated by taking the inverse of the probability of selection of each participant. These weights were adjusted for differences in the age-sex composition of the sample population as compared to the target population.

    Different weight variables are available per Step:

    • wStep1 - for interview data

    • wStep2 - for physical measures

    • wStep3 - for biochemical measures

    This allows for differences in the weight calculation for each Step of the survey as the age-sex composition of the respondents to each Step can differ slightly due to refusal or drop out.
