The Corn Production Survey (CPS) July 2009 Round Questionnaire is an eight-page form intended to gather information on corn area, production, disposition, usage of yield enhancing and protecting inputs, area and production forecast, planting intentions, and other related information. Following are the different blocks of the CPS 2009 questionnaire.

Block G - Assessment of the Farm's Corn Production
Block H - Farmers' Participation in the Ginintuang Masaganang Ani Program (GMA-Corn)
Block I - Data Collector, Supervisor, PASO and Encoded Identification
This questionnaire was used in the collection of corn production for April to June 2009, standing crop as of June 30, 2009, and planting intentions within July to September 2009. This questionnaire was also used during the April 2009 Round, October 2009 Round and January 2010 Round, with changes made on the reference periods. |
